Personally identifiable information is information about you, including name, address, email address, user ID, credit card number, social security number, etc. PEAK will never ask for your social security number or credit card information.
HOW INFORMATION IS COLLECTED, USED AND DISCLOSED
We do not collect personal information — including your email address — unless we first ask you for it. On some of our sites, we may request that you voluntarily supply us with information, including your email address, street address, telephone number or other information so that we may enhance your site visit or follow up with you after your visit. Whether you provide any information is entirely up to you. If you have voluntarily provided information, you consented to the collection and use of your personally identifiable information as described in this Privacy Statement. In some circumstances, PEAK may share information you voluntarily provide us with trustworthy business partners. We do not sell or rent personal information collected through this site to anyone. Also, PEAK may provide aggregate statistics about visitors to our sites, such as volume, traffic patterns, and related site information to reputable third-party vendors, but these statistics will include no personally identifying information. We may also disclose information in special cases when we have a good faith belief that such action is necessary to:
- Conform to legal requirements or comply with legal process;
- Protect and defend our rights or property;
- Enforce the Terms and Conditions of use; or
- Act to protect the interests of our users or others.
In order to determine a user’s ability to receive HTML email in addition to whether or not an email has been opened, Peak Performance includes a “web beacon” in a majority of the HTML email messages we send. A web beacon (actually a small .gif image) is activated when an HTML email is opened via a request for the image on our servers. This capability helps Peak Performance send email in a format users can read and enables Peak Performance to track the aggregate number of emails opened. The web beacon does not collect any personally identifiable information. Users that do not want to receive email messages with web beacons can choose to receive their emails in text (non-HTML) format. We also use the capability to track when a URL has been clicked inside of an email message sent by our system. We use this functionality to track which links were clicked and in what quantity, as well as the aggregate number of links clicked. This click tracking functionality does not collect any personally identifiable information.

If you are using a PEAK website for which you registered and choose a password, user ID or other form of authentication, we recommend that you do not divulge this information to anyone. We will never ask you for your password in an unsolicited phone call or in an unsolicited email. Also remember to sign out of the registered site and close your browser window when you have finished your work. This is to ensure that others cannot access your personal information and correspondence if others have access to your computer.
